RVP - I would highly suggest against having an open upload form like that on your website.

You are opening yourself up to someone uploading a virus to your servers. It is very simply to mask a virus as a JPG image.

Just give your webhosting a company a call and tell them what you want to do and they should offer some SAFE & SECURE ways to do it.

As is you are leaving yourself wide open.
